Only the first -Md counts. Any after that wil be ignored.

One workaround in to use the alsa "virtual midi" device. (Google for it
and you'll find a tute for setting it up.) The idea is to create a
virtual midi device and route your two controllers to that, then take
the output from the virtual midi device and use that as the input to
ecasound. Of course, you'll need to make sure your controller messages
use different channels or different controller numbers, because ecasound
will just see the one combined stream of midi events, but apart from
that, it's a usable workaround.
